 Perilymph fistula syndrome
Síndrome de fístula perilinfática espontánea

RESUMEN

El síndrome de fístula perilinfática (SFPL) incluye síntomas

de vértigo periférico, acufeno e hipoacusia. Sin embargo,

el “signo de fístula” o de “Hennebert” es lo más característico:

aparición de nistagmo al aumentar la presión en el

CAE. Estos síntomas y signos aparecen cuando existe una

comunicación entre el espacio perilinfático, que rodea al

laberinto membranoso y el oído medio, generalmente a

través de la membrana oval, redonda o cualquier anomalía

en el laberinto óseo.

Actualmente, esta situación la observamos sistemáticamente

cuando realizamos una estapedectomía. También

es frecuente como complicación de una otitis media

colesteatomatosa, siendo mucho más raro encontrarlo en

otras situaciones como por ejemplo, tras sufrir un trauma

cráneo encefálico, un barotrauma intenso, tras la realización

de un esfuerzo físico o en el curso de inflamaciones

del oído medio tanto agudas, como crónicas. Las malformaciones

congénitas constituyen el otro gran grupo de

rarezas en las que puede aparecer un SFPL.

En este artículo aportamos un caso de SFPL en una

paciente joven sin antecedentes, en la que aparecieron

los síntomas y signos de manera brusca, intensa y sin

causa aparente. Revisamos la literatura sobre el tema

SFPL encontrando numerosas publicaciones respecto a las

etiologías más comunes, pero focalizamos nuestra atención

en aquellas que han sido consideradas idiopáticas,

dudosas o de etiología desconocida, llegando a la conclusión

de que probablemente una malformación congénita

de oído interno, que ha pasado desapercibida debida a

su mínima expresión, se esconde detrás. Malformaciones

tales como anomalías en las ventanas, dehiscencias y fístulas

de canales semicirculares, tanto superior como lateral.

Destacamos las peculiaridades de su clínica, la posible

repercusión en la calidad de vida del paciente y especialmente

las posibles complicaciones como pérdida auditiva

y, mucho más grave, meningitis. En consecuencia, valoramos

las mejores técnicas de diagnóstico y la mejor actitud

terapéutica.

SUMMARY

Perilymph fistula syndrome (SFPL) includes symptoms of

peripheral vertigo, tinnitus and hearing loss. However,

the “fistula’s sign” or “Hennebert” is most peculiar:

appearance of nystagmus with increasing pressure in the

EAC. These signs and symptoms appear when there is a

communication between the perilymphatic space, which

surrounds the membranous labyrinth and middle ear,

usually through the membrane oval, round or any abnormality

in the bony labyrinth.

Currently, this situation systematically is observed when

we perform a stapedectomy. It is also common as a complication

of cholesteatoma, being much rarer to find it in

other situations such as after suffering a Traumatic Brain

Injury, an intense barotrauma or after the completion of a

physical effort, in the course of inflammation of the middle

ear both acute, as chronic granulomas etc. Congenital

malformations are the other major group of oddities that

maybe cause of SFPL.

In this article we provide a case of a young patient SFPL

with no previous history.

The symptoms and signs appeared of sudden, intense and

no clear cause. We reviewed the literature SFPL finding

numerous publications regarding the most common etiologies.

We focus our attention on those that have been

considered idiopathic, or of unknown or dubious etiology,

coming to the conclusion, that probably a congenital

malformation of inner ear, which has gone unnoticed due

to its minimal form, hides behind. Malformations such as

abnormalities in the windows, dehiscence and semicircular

canal fistula.

We highlight the peculiarities of his clinic, the potential

impact on the quality of life of patients and especially

in complications such as hearing loss and more serious

meningitis. Accordingly, we value the better diagnostic

techniques and the best therapeutic approach.
