Pemphigus vulgaris is an important autoimmune bullous
disease of the skin and the mucosas, of acute or chronic
development, and it can be life-threatening if it is not
treated with immunosuppressive drugs. They usually start
on oral mucosa, later on they appear on scalp and face,
and they progress to the neck and superior part of the
trunk.
We present three clinical cases, two gentlemen and one
lady, all of them of middle age with beginning of ENT
symptomatology.
The first patient presented a pemphigus vulgaris in the
oral cavity, but he also had a worsening of chronic renal
impairment and anaemia of chronic process. He answered
successfully to intensive corticosteroid treatment. Our
second case presented a simultaneous eruption on the
trunk and buccal mucosa and began with slight fever.
It went down slowly with immunosupressives. Our third
patient presented lesions in the oral cavity and nostrils
which did not allow deglutition, she answered rapidly to
corticosteroid therapy.
RESUMEN
El pénfigo vulgar es una importante enfermedad ampollosa
autoinmunitaria de la piel y las mucosas, de evolución
aguda o crónica, y que a menudo resulta mortal si no
se trata con fármacos inmunosupresores. Suelen iniciarse
en mucosa oral, luego aparecen en cuero cabelludo y cara,
y progresan hacia cuello y parte superior de tronco.
Presentamos tres casos, dos varones y una mujer, todos de
mediana edad con inicio de sintomatología ORL.
El primer paciente presentó un pénfigo vulgar en cavidad
oral, pero sufrió asimismo una insuficiencia renal crónica
agudizada y una anemia de proceso crónico. Respondió
bien a tratamiento corticoideo intenso. Nuestro segundo
caso presentó una erupción simultánea en tronco y
mucosa yugal y debutó con febrícula. Fue remitiendo lentamente
con inmunosupresores. Nuestra tercera paciente
presentó lesiones en cavidad oral y fosas nasales que le
impedían la deglución, respondió rápidamente a la corticoterapia
